Assisted living services are driven by a philosophy that emphasizes personal dignity and autonomy to age in place in a residential setting, while receiving increasing or decreasing levels of services as the resident's needs change. In order to meet their individual needs, the resident may require the additional services of a health care professional from a home and community support services agency. This presentation will discuss the coordination of care between the two entities, which is critical to the management of the resident’s care.
