Medicare is moving rapidly from fee for service payments to payment based on quality clincical outcomes. Through such mechanisms as ACOs, penalties on hospital re-admissions, other value based purchasing initiatives and pilot projects initiated through the Center for Medicare and Medicaid Innovations, the program is placing payment incentives for care that are based on quality patient outcomes and not quantity. Many state Medicaid programs and private insurers are following suit.
