The Texas Health and Human Services Commission (HHSC) has finalized the implementation of new Community Care rates, effective September 1, 2025, as part of the legislative direction provided during the 88th Texas Legislature. These changes impact reimbursement rates for direct care and attendant care services in Medicaid programs and conclude the legacy Rate Enhancement program structure.

What is different?

  • No minimum wage requirement effective 9-1-25.

  • ACRE has been eliminated 9-1-25.

  • Funds were appropriated to provide an average wage of $13/hour with 14% payroll tax and benefits but require no minimum wage. The total varies by program.

  • Providers will be included in an annual report to the legislature if spending for the attendant, in the aggregate, by program, is less than 90% of the attendant cost area.

Evaluation of Rate Enhancement Programs 

To support this transition, HHSC published the Evaluation of Rate Enhancement Programs 2024 report, which assesses the outcomes and fiscal impacts of enhancement payments historically used to incentivize direct care worker wage increases.
